?A Study of Regional Variants of Guillain-Barre Syndrome: A Prospective Study from a Tertiary Teaching Hospital of South India

BACKGROUND: ?The clinical spectrum of Guillain-Barre Syndrome (GBS) is ever expanding and its boundaries remain undetermined. GBS 'Variants' are so called because of close resemblance to a portion of typical illness, considerable overlap, presence of albuminocytological dissociation and absence of other causes. They suggest that the pathologic and immunologic abnormalities of GBS can be localized and selective.
DESIGN/METHODS: ?We analysed 294 consecutive patients admitted for GBS as per NINDS criteria in our institute from May 2010 to July 2012. Electrodiagnostic tests (EDX) were analysed as per American Association of Neuromuscular and Electrodiagnostic Medicine consensus report. Routine biochemical tests, CSF analysis, connective disorder profile, antiganglioside antibodies, CK, urinary porphobilinogen, serum B12, thyroid function tests were done in all patients to exclude primary cause of neuropathy. The data was separately evaluated by DP and VN. Whenever there was discrepancy, a consensus opinion was taken by collective discussion by all authors.
RESULTS: ?Of 294 patients with GBS we could identify 16 cases of GBS 'Variants'. Patients with Paraparetic Variant were 6, Pharyngeal Cervical Brachial Variant were 4 followed by Millar Fischer Variant 3, Facial Diplegia 2 and Pure Sensory Variant one.
CONCLUSIONS: ?GBS 'Variants' accounted for 8.4% of GBS patients in our institute. There was strong association with antecedent infection. There were some seasonal trends with predilection in winter and rainy seasons. PCB Variant had poorer outcome while Paraparetic Variant had better prognosis.
