Cost Analysis of an Open Source, Open Platform Teleneurology Program Designed for Rural Community Hospitals

BACKGROUND: A high percentage of patients in New Mexico lack timely access to a neurologist. As late as 2004, stroke care was well below national standards, with a TPA administration rate of 0.4%. A private tech firm was approached by the state Department of Health to develop a cost-effective telestroke platform, making full use of existing infrastructure. A pilot program was undertaken between 2/1/12 and 8/31/12 to test this system.
DESIGN/METHODS: 1. Technology. The firm had previously developed a store-and-forward system for review of CT images by the University neurosurgery department, which was installed in 20 hospitals and was found to be effective in triaging patients for transfer. This system was modified to allow for real-time video interaction and reporting. This system uses open source software and is open platform, allowing the use of off-the-shelf hardware in a secure, HIPAA compliant environment. 2. Financial analysis. Data was gathered for all telemedicine consultations over the 7 months of the pilot program. Cost and revenue estimates were provided by hospital administration and submitted to NPV analysis.
RESULTS: 114 patients were examined. 33 (29%) were stroke patients. 9 were administered TPA for a treatment rate of 27%. Eight patients (7%)were transferred to a referral center. The initial capital expenditure was $9275. Average reimbursement per patient was approximately $10,500 plus a facility fee of $85. Average cost, including telemedicine fees, was $7000. TPA increased costs by $3000 and reimbursement by $5000. Annualizing these numbers, over three years the NPV of the telemedicine project is $1.9 million.
CONCLUSIONS: An inexpensive telestroke program is financially viable for a small rural hospital.
