Comparison of Discharge Disposition of "Wake-Up Strokes" and Non Wake-Up Strokes

BACKGROUND: Wake-up strokes (WUS) are thought to have worse outcomes as compared to strokes occurring while awake, attributable to the inability of WUS patients to undergo thrombolytic therapy. We decided to assess the discharge disposition as a measure of outcome in stroke patients admitted to our center during 2010 and 2011.
DESIGN/METHODS: We retrospectively reviewed records of 438 consecutive patients with ischemic stroke from our center's stroke database. Disposition categories included: 1) discharge to rehabilitation, 2) discharge to home, 3) discharge or return to nursing home or skilled nursing facility and 4) death or hospice. We compared disposition amongst those with WUS versus non-wake up strokes using Chi-square statistics.
RESULTS: Among the 438 strokes, there were 53 patients with WUS (about 12% of all ischemic strokes). Those with WUS are more likely to be discharged to rehab than those with non-wake-up strokes (39.6% vs 25.5%), and are less likely to die (5.7% vs 14.8%) or to be discharged to nursing homes or skilled nursing facilities (9.4% vs 15.6%; X2=7.5; df=3; p=0.0579).
CONCLUSIONS: Our analysis suggests that patients with wake-up strokes may not experience worse outcomes than those with non-wake up strokes. However, these patients may still benefit from acute interventions.
