Monoclonal Gammopathy in the Full Spectrum of Motor Neuron Disorders and Multifocal Motor Neuropathy

BACKGROUND: Case reports and small case-control studies have suggested an association of monoclonal gammopathy with motor neuron disorders, but conclusive evidence is lacking. Finding such an association would suggest specific pathogenic pathways, which could lead to new therapeutic possibilities.
DESIGN/METHODS: Immuno-electropheresis and immunofixation techniques were used to determine the presence of monoclonal gammapathy in serum from 445 patients with amyotrophic lateral sclerosis (ALS), 60 patients with primary lateral sclerosis (PLS), 158 patients with progressive muscular atrophy (PMA), 88 patients with multifocal motor neuropathy (MMN) and 430 healthy controls. The presence of anti-ganglioside antibodies was determined in sera from patients with MMN and PMA and sera from patients with ALS and PLS with monoclonal gammopathy. Logistic regression analysis was used to investigate associations of monoclonal gammopathy with motor neuron disorders and clinical characteristics.
RESULTS: Prevalence of IgM monoclonal gammopathy was increased among patients with PMA (8%) (adjusted OR = 4.2; p =0.001) and MMN (7%) (adjusted OR = 5.8; p = 0.002) compared to controls (2%). High anti-GM1 IgM antibody titers were present in sera from 7% of patients with PMA and 43% of patients with MMN, but not in sera from other patient groups. None of the PMA patients with IgM monoclonal gammopathy and anti-GM1 antibodies showed conduction block or other features compatible with demyelination. The presence of IgM monoclonal gammopathy was associated with a relatively favorable disease course in patients with PMA.
CONCLUSIONS: The association of IgM monoclonal gammopathy and IgM anti-GM1 antibodies with PMA and MMN, but not with ALS and PLS, suggests that inflammatory mechanisms not only underlie MMN, but also play a role in a subgroup of patients with PMA. This would offer novel therapeutic perspectives.
