Oxidative Stress, Vascular Comorbidities and Their Impact on Dementia in the Elderly: Description and Preliminary Results of the OVID Study

BACKGROUND: Patients with VRF are at increased risk for Alzheimer's disease (AD). The reciprocal relationships between oxidative stress, vascular comorbidities and cognitive impairment are poorly understood.
DESIGN/METHODS: 250 subjects > 65 years were included in this longitudinal study. Fifty patients with mild cognitive impairment (MCI group), 50 AD patients (AD group) without vascular risk factors and comorbidities as well as 50 MCI and 50 AD patients with atherosclerosis, type 2 diabetes and/or congestive heart failure (MCI plus and AD plus, respectively) as well as 50 controls underwent physical examination, ECG, nutritional assessment, carotid ultrasound, MMSE, DemTect, Clock Drawing Test and ADAS-Cog. Patients and controls underwent a blood withdrawal for the HPLC analysis of carotenoids, tocopherols and retinol, as well as for ELISA measurements of protein carbonyls and GC/MS analysis of F2 isoprostanes as markers of lipid peroxidation.
RESULTS: MCI Plus and AD Plus patients had higher carotid intima-media thickness and higher glycemic values (p < 0.001). MCI, AD and control subjects had higher plasma levels of lutein, zeaxanthin, b-cryptoxanthin, lycopene, a-carotene, b-carotene, a-tocopherol, g-tocopherol and retinol (min. p < 0.01). Results were independent of age, gender, BMI as well as intake of fruits and vegetables. In a subset of subjects with cognitive decline after 2 years (13 MCI, 19 AD, 11 MCI Plus and 23 AD Plus) MCI Plus and AD Plus patients showed 20% higher baseline plasma levels of F2 isoprostanes compared to those MCI and AD patients with stable cognitive function.
CONCLUSIONS: Patients with VRF have lower levels of antioxidants. A complete evaluation of vascular comorbities, risk factors, and antioxidant status may help to identify patients at risk for cognitive decline and improve lifestyle- and nutrition-related counseling of patients with cognitive impairment and their caregivers.
