Stroke Reperfusion Therapy in Cook County, Illinois Is Related to Age and Race but Not Gender

BACKGROUND: We examined associations between age, gender, and race-ethnicity and use of acute reperfusion therapy for acute ischemic stroke (AIS) in Cook County, Illinois.
DESIGN/METHODS: Using the Illinois Hospital Association CompData庐, we identified patients hospitalized for AIS at Cook County's primary stroke centers (PSCs) between September 2008 and August 2011. We assessed use of RT by ICD-9 procedure codes (tPA: 99.10 and V45.88; mechanical embolectomy: 37.94). Patients were classified by age: <55 y, 55-65 y, 65-75 y, 75-85 y, and >85 y. Within combined age, race/ethnicity and gender subgroups, we assessed use of RT.
RESULTS: There were 18,474 AIS discharges and use of RT was 5.8%. RT differed by race (B: 4.8% vs. W: 6.7% and H: 6.2%; p<0.001), age groups(< 55 y: 7.7% vs. other groups: 5.5%: p<0.001) but was similar by gender (women: 5.9% vs. men: 5.7%, p=0.723). Among women, black and Hispanics 75-85 y were less likely to receive RT than whites (B: 2.9% and H: 2.5% vs. W: 8.1%, p<0.001). Among men, blacks < 55 y were less likely to receive RT (B: 6.3% vs. H: 10.0% and W: 9.6%; p=0.049) and blacks and Hispanics aged 65-75 y were less likely to receive RT than whites (B: 3.3% and H: 4.1% vs. W: 6.5%; p=0.017).
CONCLUSIONS: Age-related differences in stroke RT were seen in specific race/ethnic and gender subgroups. Compared to whites < 55 y and 65-75 y, black and Hispanic men were less likely to receive RT while black and Hispanic women in the 75-85 y age group less likely to receive RT than whites. Our data suggest that race/ethnic barriers for acute stroke therapy may differ by age and gender subgroups.
