Title Efficacy of Tolcapon on Parkinson's Disease Associated Non Motor Symptoms: The TANIMOS Study

Objective

Background Addition of Catechol-O-Methyltransferase inhibitors to a conventional levodopa/dopadecarboxylase inhibitor regime improves motor symptoms in PD patients, suffering from wearing off. Motor fluctuations are related to onset of certain non motor features.

Design/Methods 125 patients (75 male) treated with conventional levodopa formulations additionally took tolcapone in this observational open trial. Their concomitant dopaminergic substitution therapy should remain stable, but drug- and dosage changes were documented. Initially and following four weeks of adjunctive tolcapone (100 mg t.i.d.) intake, patients were scored with UPDRS parts I, II, IV, the non-motor symptoms scale for Parkinson's disease (NMSS) by their neurologist. He also recorded the daily off time. The patients rated themselves with the EuroQuol (EQ-5D), the EQ-5D visual analogue scale (VAS) and the non motor screening questionnaire (NMSQuest). Caregivers reported the daily duration of care giving (DDCG) in various domains.

Results UPDRS I (initial: 3.67 ± 0.25; end: 2.74 ± 0.23 [mean ± SEM]), II (initial: 17.04 ± 0.79; end: 13.48 ± 0.76), NMSS (initial: 55.77 ± 3.49; end: 40.05 ± 3.23 [all domains improved except perceptual problems/hallucination and attention/memory]), NMSQuest (initial: 10.65 ± 0.51; end: 8.16 ± 0.44), OFF Time (initial: 186.4 ± 17.58; end: 88.91 ± 8.79 [min]), DDCG (initial: 389.9 ± 49.47; end: 308.3 ± 44.15 [min]), EQ-5D (initial: 9.02 ± 0.17; end: 8.06 ± 0.2), VAS (initial: 46.65 ± 1.57; end: 55.39 ± 1.7) significantly improved, but not UPDRS IV (initial: 6.94 ± 0.44; end: 6.43 ± 0.43). There were no clinical relevant liver enzyme elevations.

Conclusions This is the first trial that demonstrates the efficacy of tolcapone addition on the appearance of non motor symptoms in PD patients by neurologists, the patients themselves and their caregivers.
