Use of ACEI in Migraine Prophylaxis: Why We Don't Look Back?

However with the higher association between migraine and stroke and the projection of increase in stroke in the future years it is worth to consider the use of ACEI for patients with migraine headaches and hypertension and even the use of these medications as a prophylactic agent in patients with only migraines. Molecular biology has showed the components of the brain rennin-angiotensin system (RAS) in the brain which may play a role in the pathogenesis of migraine.
Case description of a 32 year old female with grade I hypertension and one episode of migraine without aura since age 13 with 1 episode per week who after the initiation of monopril 10 mg presented a reduction of migraine headaches to one episode per month (associated to her menstruation). Patient reported no side effects from the medication after one year of the initiation of the above mentioned dose.
ACE inhibitors modulate vasoreactivity, alter sympathetic tone, promote degradation of proinflammatory factors, such as substance P, encephalin, and bradykinin, decrease free radicals, increase protacyclin and could also modulate the endogenous opioid system which may explain its role in decreasing migraine headaches.
ACEI have a lower profile of side effects compared to other blood pressure medications currently used in migraine prophylaxis. The benefits with ACEI recognized in the management of congestive heart failure, hypertension associated with diabetes and secondary stroke prevention maybe can be extrapolated to a younger population. Early initiation of an ACEI in patients with migraine attacks can possibly reduce the incidence of stroke in a population at a higher risk of it later in life.
