Characterization of Cognitive Profile for Asian Patients with Multiple Sclerosis and Neuromyelitis Optica

BACKGROUND: Between 40-65% of MS patients have cognitive impairment, which adversely affects personal, social and occupational functioning. While a broad range of cognitive deficits have been described in MS, cognition in NMO is less clearly understood. Few studies of cognition in demyelinating diseases exist in Asian populations.
DESIGN/METHODS: Patients and controls were recruited from the NNI Neuroimmunology clinic. Demographic, clinical and Expanded Disability Status Scale (EDSS) information were recorded. Memory (immediate/delayed/recognition recall), executive function (Frontal Assessment Battery), attention/working memory (digit span, Paced Auditory Serial Addition Test), and information processing speed (Symbol Digit Modalities Test, Color Trails 2) assessments were done. Standardized cognitive domain scores were derived from these tests. Participants also completed self-rated questionnaires evaluating fatigue, apathy, anxiety and depression.
RESULTS: Seventy-eight participants (40 MS, 12 NMO and 26 controls) underwent testing. Mean age was 45.48 years (SD=11.95); mean duration of education was 13.32 years (SD=3.32). EDSS was comparable between MS and NMO patients [1.54 vs 2.17,p=0.076]. ANCOVA analysis of MS vs NMO vs controls, adjusting for age and education, showed a significant difference in attention/working memory [Mean(SD):-0.16(0.82) vs -0.08(0.76) vs 0.26(0.83); F(2,69),p=0.007], and a trend for improved information processing speed [-0.09(0.46) vs -0.06(0.37) vs 0.14(0.41); F(2,68),p=0.073]. In MS, overall cognition (average of standardized cognitive domain scores) was negatively correlated with EDSS (r=-0.32,p=0.056), with executive function demonstrating the strongest correlation (r=-0.43,p=0.006). No differences were found in fatigue, apathy, anxiety and depression ratings.
CONCLUSIONS: Attention/working memory and processing speed are most impaired in MS compared to NMO patients and healthy controls. Executive dysfunction is strongly correlated with more severe disability. Further investigation into the effects of illness duration and other contributing factors on cognitive impairment is under analysis.
