How Close Are We from Achieving Prevention Target Goals after Stroke?

BACKGROUND: Current guidelines recommend a set of target goals for effective secondary prevention after ischemic stroke. However, little is known about the rate of achievement of these target goals outside clinical trials. This study aims to estimate the proportion of patients who achieved such target goals.
DESIGN/METHODS: We conducted a retrospective cohort study by identifying patients admitted to King Abdul-Aziz medical city in Riyadh from 2007 to 2009 due to ischemic stroke. Clinical and laboratory characteristics were reviewed. Last posted visits were reviewed from six weeks after discharge onward. We collected the last observations of blood pressure, LDL level, HbA1c from electronic records. The target goals were blood pressure (BP) ?130/85 mmHg, LDL <1.7 and HbA1c <0.07. Study was approved by local IRB.
RESULTS: Among 306 survivors, the mean age was 68.9 +/-12 SD, 69% were males. At baseline, 79.7 % had HTN (hypertension), 63.4% DM (diabetes), 28.4%DLP (dyslipidemia). After a mean of 15 months (+/- 4.3) follow up, at least single BP measurement was found in 83.3% of patients, LDL 57.2% and HbA1c 50.3%. Considering those with no follow up data as if they did not achieve the target goals, 40.2 % achieved BP, 24.8 % HbA1c, 15 % LDL target goals. Development of in hospital complication was associated with OR 4.488 (95%CI=1.162-17.339) for predicting all target goal achievement.
CONCLUSIONS: Secondary prevention of ischemic stroke is sub optimal. Significant portion of our cohort did not achieve the target goals as per guidelines. Those who developed in hospital complications had better goal achievement on subsequent follow up. keywords :Stroke secondary prevention, risk factor control, Guidelines goal achievement.
