Histopathologic Diagnostic Review in a Cohort of Patients with Primary Central Nervous System (CNS) Tumors in a University Institute

BACKGROUND: Histopathological diagnosis discrepancy was observed in several neurooncologic studies on primary CNS tumors. This discrepancy was also observed between external Pathology Reports (PR) and those performed by the Pathology Department of our institution.
DESIGN/METHODS: This is a clinical, descriptive, observational and retrospective research study of 275 medical records of patients diagnosed with primary CNS tumor in the period between January 2002 and December 2011 in a monovalent University Institute. We compared the PR from other centers and those that emerged from the review of the pathological material in our institution, as classified by the World Health Organization. The outside histopathological diagnosis centers were classified in levels according to the guidelines of the organization and operation of the Services of Pathology and Laboratory of Pathology of the Ministry of Health of Argentina.
RESULTS: Seventy-eight PR were collected from 74 patients. We observed diagnostic concordance in 62 PR (79%); in 35 of the external PR (56%) immunohistochemistry (ICH) was mentioned. Tumor subtyping was performed without changes in therapeutic behavior in 17/62 cases. The discordant PR were 16 (21%), and in 10 of these ICH was not performed. In 14/74 cases (19%) the final diagnosis modified the therapeutic treatment. Diagnosis dominated discordant in lower level institutions without ICH.
CONCLUSIONS: The review of biopsy specimens is warranted in all patients, including those from high complexity centers. Diagnostic difficulties do exist, and referrals are important in case of diagnostic discrepancies.
