BACKGROUND: Epilepsy induced by mitochondrial encephalomyopathy is a special type of epilepsy caused by mtDNA or nDNA deficiency and mitochondrial obstacle of structure and function. Because of lack of awareness of this disease in the past,we could not choose an appropriate drug to treat this type of epilepsy.
DESIGN/METHODS: A randomized, open-label, controlled, parallel group, multicenter trial was conducted to test the superiority of the traditional AED arms over the newer AED arms. The primary endpoint was the rate of seizure-free patients in the first 12 weeks. Furthermore, efficacy, tolerability and quality of life were evaluated. The objects included 84 patients aged ?18 years diagnosed epilepsy induced by mitochondrial encephalomyopathy(partial or secondary generalised epilepsy). Patients were titrated to 2000 mg/day of LEV or 200 mg/day of TPM or 300 mg/day of CBZ or 1000 mg/day of VPA respectively.
RESULTS: The proportions of seizure-free patients were 57.1%(LEV),42.9%(TPM),57.1%(CBZ) and 4.8%(VPA) during the whole treatment period of 52 weeks. Among three groups of LEV,TPM and CBZ,there were no significant differences (P>0.05).But VPA group had no effect. Adverse events occurred in 57.1% (LEV),71.4% (TPM) 57.1% (CBZ) and 95.2%(VPA) of the patients. Among the three groups there were no significant differences (P>0.05).But VPA group had significant side effect(P<0.01).Adverse events associated with study discontinuation occurred in 1/21 (LEV) ,3/21 (TPM) ,0 (CBZ) and 19/21(VPA) patients.
CONCLUSIONS: There were no significant differences with regard to efficacy and tolerability of LEV,TPM and CBZ in newly diagnosed epilepsy induced by mitochondrial encephalomyopathy.But VPA can not used for these patients because it had no effect and had a series of side effects.
