Title Contribution of CSF to the Diagnosis of McDonald MS: Retrospective Analysis in the BENEFIT Study

Objective

Background BACKGROUND: The McDonald criteria were revised (2005 and 2010) to improve sensitivity for early MS without compromising specificity. In the 2010 version (MDMS2010), dissemination in time (DIT) can be fulfilled in one single scan and, for dissemination in space (DIS), the Swanton Criteria replaced the Barkhof criteria. Also, the CSF criterion (presence of oligoclonal bands or increased IgG) was not included. However, the role of CSF in the initial diagnosis of MS is still disputed and of practical relevance.

Design/Methods DESIGN/METHODS: MDMS2010 vs MDMS+CSF (accepting the 2001/2005 CSF criterion to support DIS) were applied to patients with clinically isolated syndrome randomized in the BENEFIT trial of early vs delayed treatment with interferon beta-1b.

Results RESULTS: Baseline CSF data were available for 314 of the 468 patients in the original cohort, and 267 (85%) were CSF+. At baseline, 109 patients (34.7%) met MDMS2010 criteria and 122 (38.9%) MDMS+CSF. The proportion of patients (Kaplan-Meier estimates) at Year 1 who met MDMS2010 vs MDMS+CSF was 71.0% vs. 76.8% and at Year 5 85.3% vs. 89.0%. Of the 13 CSF+ patients who did not meet MDMS2010 criteria at baseline 7 converted to clinically definite MS (CDMS) by Year 5. The hazard ratio for 5-year progression to CDMS was 1.275 [0.918-1.771] for MDMS2010 and 1.285 [0.930-1.774] for MDMS+CSF at baseline.

Conclusions CONCLUSIONS: Overall, the results showed the inclusion of CSF criteria is still of practical relevance for the diagnosis of MS. Approximately half of the CSF+ but MDMS negative patients at baseline converted to CDMS within 5 years. The additional inclusion of CSF criteria to MDMS2010 may increase sensitivity without compromising specificity.
