Title Effects of Fingolimod on Disability Progression in Patients with Disability as Measured by EDSS at Baseline: Post-Hoc Analyses of FREEDOMS I and II

Objective

Background BACKGROUND: Efficacy of fingolimod has been assessed in two placebo-controlled trials, FREEDOMS I and II. Subgroup analyses have shown that patients with a baseline EDSS score of 0 are subject to considerable measurement variability, and potential random error in estimating time to confirmed disability progression.

Design/Methods DESIGN/METHODS: Post-hoc analyses of disability endpoints in FREEDOMS and FREEDOMS II were conducted in patients with an EDSS baseline score greater than 0, and contrasted with the corresponding analyses in the overall population. Endpoints included time to 3-month and 6-month confirmed disability progression as measured by EDSS.

Results RESULTS: Fingolimod 0.5 mg reduced the risk of disability progression, confirmed after 3 months, over the 24-month study period, by 37% and 30% (hazard ratios, 0.63; p:0.01 and 0.70; p:0.04) in FREEDOMS and FREEDOMS II. Regarding disability progression that was confirmed after 6 months, the risk was reduced even further, and consistently across both studies (hazard ratios: 0.53 and 0.54, respectively, p:<0.05).

Conclusions CONCLUSIONS: These post-hoc analyses suggest that fingolimod is efficacious in delaying disability progression in patients with an EDSS score at baseline greater than 0. Results were consistent across the two phase 3 trials. Analyses of 6-month confirmed disability progression suggest that patients treated with fingolimod may have their risk of further progression reduced by almost half compared to patients treated with placebo.
