Microglial Microvesicles: Useful Prognostic Tool for Mild Cognitive Impairment and Alzheimer Disease Patients

BACKGROUND: Microglial microvesicles have been indicated as important mediators of intercellular communication and are emerging as new biomarkers of neuroinflammation and neurodegeneration. Many studies using PET techniques on Alzheimer disease have recently demonstrated the role of microglia in identifying MCI converters from Healthy Controls and monitoring AD progression. Our previous study on AD patients already showed an increase of MMVS levels in comparison to MCI and Healthy controls.
DESIGN/METHODS: 60 patients affected by MCI and 110 affected by AD were recruited at San Raffaele Hospital and evaluated every 6 months in a 3years-study.Patients underwent at diagnosis CSF, MRI and neuropsychological assessment analysis. 200 uL of CSF were analyzed by flow cytometry (Annexin V, IB4) to detect Microglial Microvesicles while 200 ul of CSF were used to test cytokines (IL1b, IL6, CCL2, TNFalfa, CCL17, CCL22, YKL-40) by gene expression assays.
RESULTS: Patients with a rapidly progressive form of AD show higher MMVs level, with a correlation with M1 cytokines and and inverse correlation with CSF Beta-amiloid. Also MCI patients that converted to AD show higher MMVs level, but no correlation with the cytokine profile or with other CSF markers was evident. Neuroradiological analysis (DTI, fMRI) are ongoing.
CONCLUSIONS: Microglial microvesicles may be used as markers for monitoring neuroinflammation in dementia patients. Our finding suggest that microglial activation occurs even in the earlier stages of the disease and may be an useful tool for stratifying patients at increased risk of rapid progression of disease.
