Title Outcome, Recurrence and Mortality after Non-Valvular Atrial Fibrillation Stroke. Long-Term Follow-Up Study

Objective

Background BACKGROUND: NVAF is a major risk factor for IS and a powerful predictor of mortality. Several studies reported higher case fatality and morbidity after acute IS among NVAF patients; however the long-term follow-up was poorly studied.

Design/Methods DESIGN/METHODS: We analyzed data from a prospectively collected database at the stroke clinic at a tertiary hospital in Mexico City, over a period of 20 years. Poor outcome (Modified Rankin Score ?3), recurrence, mortality and risk factors were analyzed with multivariate analysis adjusted by age.

Results RESULTS: 690 patients (572 with LAA and 118 with NVAF), were included (390 males, 300 females with a mean age 67.66±9.5). NVAF group were predominantly males (56.5%), older (67 vs. 70 years), with worst 3 to 6 months and 6 to 18 months outcome (57.5 vs. 69.6% and 57.6 vs. 69% respectively). After a median follow-up of 29 months (range 0-261) we observed a trend in worst outcome (57 vs. 72.6%; p=0.240), IS recurrence (23.9 vs. 18.1%; p=0.155) and deaths (26.1 vs. 14.6%; p=114) in NVAF group. Patients with LAA had significantly more hypercholesterolemia (54 vs 33%), diabetes (34.8% vs 29.3%) and tobacco use (40.6 vs. 21.2%). These differences were not significant after adjusted by age.

Conclusions CONCLUSIONS: We found significant poor short and middle term outcome, trend for more recurrence and death in NVAF stroke patients; these differences continue only as a trend in the long-term follow-up. Our findings highlight the importance of appropriate primary and secondary prevention in NVAF stroke.
