Increased Fasting Glucose, 2-h, 3-h and 5-h OGT Serum Glucose Levels Are Associated with Neuropathic Pain

BACKGROUND: Between 13 and 21% of individuals with prediabetes suffer of neuropathic pain. Different screening methods have reproduced different results.
DESIGN/METHODS: A brief neuropathic screening questionnaire was applied before patients underwent simultaneous 12-hour fasting glucose and 2, 3 or 5-hour oral glucose tolerance (OGT) tests. In order to compare more than a single value and to show the total exposition to different levels of glucose, the area under the concentration-time profile for glucose response curve of the OGT was calculated for the 3-hour and 5-hour OGT tests. None of the patients had peripheral neuropathy, impaired glucose tolerance or diabetes diagnosed before the test.
RESULTS: Fifty-five patients (37 women) were analyzed, with a mean age of 45 years (range: 18-78 years). Thirty-four (60.7%) OGT yield normal results (94卤18.4mg/dl), 19 (33.9%) had glucose intolerance (155卤21.8), and 3(5.4%) had overt diabetes (262.67卤59mg/dl). Of all the patients, 32(58.2%) had paresthesias, 13(23.2%) had neuropathic pain and one (1.8%) had numbness. When patients with and without neuropathic pain were analyzed, there were no significant differences in age (43卤14 vs 49卤12 years old). However, two-hour glucose serum levels were significantly higher in patients with neuropathic pain (147卤60.8 mg/dl vs 117卤43.29 mg/dl, p=0.048). Twelve-hour fasting glucose (102卤14.3 vs 98卤10 mg/dl), the area under the curve in the 3h-OGT (23,268 vs 22,054 mg/dl) and 5-hour OGT (42,510 vs 29,837 mg/dl) were also higher. Glucose levels were similar in patients with and without other neuropathic symptoms as paresthesias, numbness, or muscle weakness.
CONCLUSIONS: The mean 12-hour fasting glucose levels, the 2-hour OGT levels and the total area under the curve for the 3 and 5-hour OGT tests were higher in patients with neuropathic pain. The presence of paresthesias, numbness or muscle weakness did not correlate with any glucose serum level measurement.
