Associations of Race with Self and Informant Complaints of Cognitive Difficulties in a Community Based Sample of Non-Demented Elders

BACKGROUND: Though self- and informant reports of cognitive difficulties are essential to the diagnosis of dementia, they have not been investigated in diverse groups.
DESIGN/METHODS: The Einstein Aging Study (EAS) is a community-based longitudinal study of Bronx adults 70+. Self- and informant reports of cognitive status were assessed using select items from the CERAD clinical history questionnaire, Albert Einstein Health Self-Assessment Form, and Geriatric Depression Scale, short form (GDS). We used baseline data from the Einstein Aging Study to assess the rates and associations of self and informant complaints (each was stratified into low, medium, or high rates) in blacks and whites. Using logistic regression models, we adjusted for age, gender, education (>12 years vs. LE to 12 years), memory status, measured by the Free and Cued Selective Reminding Test (dichotomized as free recall scores <27 versus >=27). In a second model we also adjusted for depression (dichotomized as GDS scores <7 versus >= 7).
RESULTS: The sample included 1729 individuals (27% black; 61% female). Using the lowest level of self-reported complaints as the reference group, blacks were more likely than whites to be in the medium (OR=1.30, CI: 1.01-1.67, P=0.04) and high complaint groups (OR=1.37, CI: 1.02-1.846, P=0.04). Results were not materially altered by adjustment for depression. An opposite, though non-significant pattern was found for informant complaints (n=908), where black race was associated with lower levels of informant complaints (OR=0.82, CI: 0.61-1.10, P=0.1911).
CONCLUSIONS: Non-demented black older adults showed significantly higher level of self-reported memory complaints than whites even after adjusting for age, gender, education, and memory. Informant complaints show no significance difference between racial groups. The predictive validity of these complaints will be assessed in our longitudinal study.
