Fluctuations of Plasma Microbial Translocation Products among Patients at Risk of Chronic HIV-Associated Neurocognitive Disorder

BACKGROUND: Cognitive impairment continues to be an important manifestation in about 15-50% of HIV-infected subjects despite of effective anti-retroviral therapy. Circulating lipopolysaccharides (LPS), derived from the gastrointestinal tract as a result of microbial translocation, is elevated in patients with chronic HIV infection compared to uninfected patients. LPS induces the release of pro-inflammatory cytokines, monocyte activation and enhanced trafficking into the brain all considered to be important mechanism in the pathogenesis of HAND. The degree of MTPs fluctuation within a subject is not known.
DESIGN/METHODS: Two groups of subjects, viremic (undetectable viral load) (n=22) and aviremic (viral load >500 copies /mL) (n=8) were recruited. Blood was collected at 6 time points during the 4-week study. A questionnaire of inter-current illness, anti-biotics use, alcohol intake was administered at each of the 6 visits. Blood samples were analyzed for lipopolysaccharide (LPS), lipopolysaccharide binding protein (LBP), and endotoxin core antibodies levels. Fluctuations of MTPs within individual were calculated as the standard deviation (SD) of all MTPs readings during the study period.
RESULTS: The SD of LPS for aviremic subjects ranged between 0 - 12.2 EU/mL, with mean SD of 1.5EU/mL, compared to 0.092- 2.71EU/mL, and mean SD of 0.66 EU/mL in the viremic subjects. The SD of LBP in the aviremic group ranged between 983-49730 ng/mL, with a mean of SD of 7366 ng/mL, while the viremic group ranged between 1993- 12348ng/mL, with a mean of 5455 ng/mL.
CONCLUSIONS: There were significant fluctuations of MTPs within individuals, and the magnitude of fluctuation may trend higher in aviremic subjects.
