Neuroanatomical Correlates of Emotional Blunting in Behavioral Variant Frontotemporal Dementia

BACKGROUND: Patients with bvFTD, who demonstrate prominent atrophy in orbitofrontal and anterior temporal regions, often exhibit signs of emotional blunting. We used tensor-based morphometry (TBM) to map the association between regional brain volumes and caregiver ratings on a scale of emotional blunting.
DESIGN/METHODS: Caregivers of 9 study participants with bvFTD (mean age=63.2, sd=10.2) completed a questionnaire assessing symptoms of emotional blunting (EB). TBM was applied to 3D T1-weighted MRI scans for each participant. Each individual image was warped to a common brain template, and Jacobian maps showing the expansion or compression factor at each voxel were created. Mean Jacobian values within the orbitofrontal and anterior temporal regions were computed for each subject to provide a numeric summary of regional volume. Voxel-wise regression of EB ratings was performed on the Jacobian maps.
RESULTS: EB ratings were significantly associated with anterior temporal lobe volume on the right (r=-.74, p=.02), but not on the left (r=-.17, p=.67). The association between EB and orbitofrontal volume was also stronger on the right (r=-.39, p=.30) than on the left (r=-.07, p=.85), but neither correlation was statistically significant.
CONCLUSIONS: Symptoms of emotional blunting in bvFTD patients appear to be particularly associated with right-sided atrophy, with significant involvement of the right anterior temporal region, but not the orbitofrontal cortex.
