Web-Based Assessment of Functional and Quality of Life Outcomes after Intracranial Hemorrhage

BACKGROUND: The modified Rankin Score (mRS), a validated score from 0 (no symptoms) to 6 (dead), is assessed commonly by researchers over the phone or in person using a validated questionnaire which can be time-consuming and inconvenient. We tested the hypothesis that patients and proxies could accurately report their mRS and QOL outcomes on the web compared to a mRS assessed by study staff.
DESIGN/METHODS: We prospectively enrolled patients with spontaneous intracerebral hemorrhage (ICH) or aneurysmal subarachnoid hemorrhage (SAH). Patients,or a proxy, were contacted for follow-up at 28 days, 3 months, and 1 year by study staff to score the mRS. The patient or a proxy separately assessed the mRS on a secure, purpose-built website and completed a computer-adaptive test to score mobility from Neuro-QOL (a validated assessment). mRS scores from study staff and website reporting were compared with Spearman's rho and Pearson's kappa. Both mRS scores were compared to Neuro-QOL mobility scores reported by the patient or proxy with ANOVA.
RESULTS: Our analysis included 59 patients (26 ICH and 33 SAH patients) with 88 assessments. There was very good agreement on the mRS between researcher and patient/proxy assessments (rho=0.82; kappa=0.6; p<0.001 for both). mRS scores (from study staff or patient/proxy) were associated linearly with Neuro-QOL mobility scores (P<0.001 for both).
CONCLUSIONS: Reasonably accurate mRS scores for ICH and SAH patients can be obtained from patients (or proxies) online without an interview and both correlate with validated mobility scores. Web based outcomes ascertainment is a feasible and accurate option for follow-up assessments in clinical research.
