The Anatomical Variation between the Superficial Radial and Lateral Antebrachial Cutaneous Nerves. A Nerve Conduction Study

BACKGROUND: The interpretation of electromyography (EMG) tests in cases of focal lesions of peripheral nerves is based on detailed knowledge of nerves and muscles anatomy. In patients with RSN-LACN variation, the EMG examination may be impaired in the evaluation of proximal lesions of the radial nerve (RN) in the arm, lesions of the distal RSN in the wrist and in LACN injuries. Especially in cases of proximal RN injury, the occurrence of this variation may lead to diagnostic errors in the EMG study, with a total axonal injury, erroneously assessed as partial lesion with conduction block component.
DESIGN/METHODS: We have performed nerve conduction studies in 200 upper limbs of 100 patients divided into two series of 50 patients. We seek evidence of this variation on the dorsum of the hand and in the thumb. The Institutional Review Board approved the clinical research and we obtained informed consent from all subjects.
RESULTS: In 100 upper limbs studied for the occurrence of this variation on the dorsum of the hand we found 8 variants upper limbs. In 100 upper limbs studied for the occurrence of this variation on the thumb we found 11 variants upper limbs.
CONCLUSIONS: The anatomical variation between the RSN and LACN can be study by nerve conduction. The frequency of occurrence of this poorly known variation makes his knowledge important in the clinical practice of electromyography.
