Objective

Background BACKGROUND: Spinal dural arteriovenous fistulas are the most common type of arteriovenous malformation involving the spinal cord, especially in middle-aged men.

Design/Methods DESIGN/METHODS: We present the case of a 71-year-old man with progressive paraparesis and urinary urgency over a year. An spinal MRI revealed T9-T10 intramedullary signal changes with dilated vessels on the dorsal cord that suggested a spinal dural AV fistula. An initial catheter-based spinal angiogram was limited to evaluating the cervical and thoracic levels without lumbar exploration due to excessive tortuosity as well as high radiation and contrast burden. Subsequently a multi-phase 3D fluoroscopic-triggered elliptic-centric contrast enhanced MRA was performed in the sagittal plane of the lumbar spine which enabled optimal opacification of the abdominal aorta, radicular arteries, the AV fistula and draining veins. Each phase lasted 58sec. with a total scan time of 3min. The scan parameters were 340cm FOV with a matrix of 320x320 resulting in 1.1 x 1.1 mm in plane resolution with , 1.6mm slice thickness reformatted to .8mm thickness. Following this post contrast sequence opacification of abnormal dilated medullary veins posterior to the thoracic cord with two additional abnormal epidural veins at the T11 level and L3 vertebral body levels. An enlarged venous varix of was visualized at the L3 level.

Results RESULTS: Subsequently underwent super selective exploration of his lumbar segmental artery, with successful embolization with minimal contrast.The diagnosis & localization of vessel was established by MRA which reduced the total dye burden of procedure.

Conclusions CONCLUSIONS: 3D contrast enhanced spinal MR angiography is a safe, effective, noninvasive procedure that allows clear visualization of the AVM providing detailed depiction of the vascular anatomy of spinal vessels. However, further studies will be needed to assess the efficacy of these modalities in decreasing surgical risk.
