Post-Stroke Aphasia: Differences in White Matter Fractional Anisotropy between Recovered and Non-Recovered Patients

BACKGROUND: LMCA stroke often results in language deficits in addition to damage in left hemispheric regions and their connections. Diffusion tensor imaging (DTI) allows examination of differences in white matter fractional anisotropy (FA) between individuals who recovered language function ?1 year following LMCA stroke and those who remained aphasic.
DESIGN/METHODS: 27 LMCA stroke subjects ?1 year post-stroke underwent anatomical T1-weighted MRI and 32-direction DTI. Language ability was categorized based on their Token Test performance: 9 scored in the normal range (?41); 18 were considered aphasic (<41). Images were pre-processed in AFNI including co-registration of anatomical and DTI scans, motion and eddy current distortion correction, and tensor decomposition to create individual subject FA maps. Tract-Based Spatial Statistics (TBSS) method in FSL was used to prepare subject FA maps for group analysis. FA threshold was set to ?0.3 to restrict analysis to white matter. Two-sample t-test was performed to compare white matter FA of patients with and without aphasia; Threshold Free Cluster Enhancement (TFCE) method was used to correct for multiple comparisons.
RESULTS: We observed overall greater FA in the left hemisphere white matter of non-aphasic patients compared to aphasic patients (corrected p<0.05), including in the corticospinal tract, inferior longitudinal fasciculus, inferior fronto-occipital fasciculus, internal capsule, superior longitudinal fasciculus and the forceps minor. Increased FA in non-aphasic compared to aphasic patients was also widespread in the corpus callosum (corrected p<0.05).
CONCLUSIONS: Greater left hemispheric damage to brain structures and decreased intrahemispheric connections in patients with aphasia are evident in the white matter FA differences between the groups. Group differences in corpus callosum FA also indicate possible reductions in interhemispheric connections in chronically aphasic patients. These group differences may underlie post-stroke recovery.
