"Mild" Sleep Disordered Breathing Has Significant Implications for the Treatment of Many Neurological Symptoms

BACKGROUND: Previous studies have suggested a relationship between several neurologic symptoms including headache, cervical radiculopathy, lumbar radiculopathy and excessive daytime sleepiness and obstructive sleep apnea (OSA). However, these same symptoms could be associated with a milder form of sleep disordered breathing, UARS.
DESIGN/METHODS: This study is based on a retrospective review of 3034 consecutive patients who underwent polysomnography while being evaluated in a general neurology outpatient clinic. An odds ratio (OR) analysis was performed using primary symptoms, and UARS, and an innovative technique for visualization of the data was developed to depict these associations.
RESULTS: A RERA Index >=10 and Apnea-Hypopnea Index< 5 (definition of UARS) and primary/secondary variables yielded the following odds ratios: Sleep Related Movement Disorder (SRMD) - 12.3; Dizziness - 9.2; Fibromyalgia - 9.0; Migraine - 8.4; General Headache - 8.3; Cervical Radiculopathy - 7.9; Lumbar Radiculopathy - 5.8; Multiple Sclerosis - 5.3; and Restless Leg Syndrome (RLS) - 5.0.
CONCLUSIONS: Many neurologic symptoms including Sleep Related Movement Disorder (SRMD), Dizziness, Fibromyalgia, Migraine, General Headache, Cervical Radiculopathy, Lumbar Radiculopathy, Multiple Sclerosis, and Restless Leg Syndrome (RLS) can be associated with mild sleep disordered breathing. The reason for these associations must await further evaluation.
