Vitamin D Levels in African-Americans and Caucasian with Relapsing Multiple Sclerosis: Relationship with Disease Course

BACKGROUND: Vitamin D, a secosteroid, may favorably affect disease course in MS. Additionally, vitamin D metabolism and serum levels are affected by multiple factors including diet and exposure to sunlight. It is well-known that AA-MS patients experience a more aggressive disease course than CA-MS. We investigated the relationship between vitamin D and disease course in AA-MS compared to CA-MS.
DESIGN/METHODS: This was a cross-sectional study design. Records were obtained from our MS Center with well-characterized MS patients. Only relapsing MS patients (RR or SP) were included. Moreover, only those patients were included who were not taking or had not previously received vitamin D supplements at the time of measurement of vitamin D levels.
RESULTS: 100 consecutive patients in the AA-MS and CA-MS groups each, were included in the study. Mean age, disease duration, and time to onset of Rx were similar in the groups. However, mean EDSS were 4.3 (2.0) in the AA-MS and 3.1 (1.9) in the CA-MS (p<0.0001) and mean T2LL were 18.12 (11.08) ml and 13.32 (8.01) ml (p<0.0001) in the AA-MS and CA-MS, respectively. Mean vitamin D levels were 16.6 (6.8) in the AA-MS and 23.7 (8.6) ng/ml in the CA-MS patients (p<0.0001). Vitamin D levels showed significant inverse correlation with EDSS and T2LL in both patient groups. Additional analyses with multiple regression will also be presented.
CONCLUSIONS: AA-MS patients demonstrate significantly lower and deficient vitamin D levels compared to CA-MS patients. Low vitamin D level is associated with higher disability and MRI disease burden disease in both groups. Lower vitamin D levels in AA-MS may partly contribute to a worse outcome, well-described in this population with MS.
