The Prevalence of Incidental Findings on Magnetic Resonance Imaging Studies in Multiple Sclerosis Patients

BACKGROUND: The issue of reporting and managing incidental findings has important implications for patient safety; is integral to the informed consent process in clinical research. Magnetic resonance imaging (MRI) is widespread in multiple sclerosis (MS) studies, yet the prevalence rate of incidental findings in MS is unknown.
DESIGN/METHODS: This was a cross sectional study of 120 MS patients and 60 age and sex-matched controls. During the consent process, patients were informed that they would be made aware of incidental findings by the principal investigator.
RESULTS: The MS subgroup (mean age 45. 8 years; 74 males) included 89 relapsing remitting, 12 primary progressive, 15 secondary progressive, 3 clinically isolated syndrome, and 1 Neuromyelitis Optica patient. The median Expanded Disability Status Scale score was 3.0 (0 - 7.5) and the mean disease duration was 12.9 years (range 1 - 40 years). To date, 23 subjects were found to have incidental abnormalities detected on study brain and neck MRI, including: thyroid nodules (6 controls, 4 MS patients), pituitary lesions (1 control, 1 MS patient), a subclinical stroke (1 control), cavernomas (2 controls, 3 MS patient), enlarged cervical lymph nodes (1 MS patient), a disc protrusion (1 MS patient), a thoracic spinal meningioma (1 control), and 2 aneurysms (2 MS patients). All subjects were made aware of significant findings by the principal investigator, with appropriate management initiated.
CONCLUSIONS: Given the frequency of MRI brain incidental abnormalities in our study cohort amongst our study participants, we suspect such findings are common. Improved patient enrolment and relationships are likely to stem from a transparent approach to these unexpected results.
