Aortic Arch Calcification, Procedure Times, and Outcomes of Carotid Angioplasty and Stent Placement

BACKGROUND: Severe aortic disease can add considerable complexity to accessing supra-aortic arteries as part of carotid angioplasty and sent placement (CAS). Aortic arch calcification is frequently seen in patients as a marker of aortic atherosclerosis.
DESIGN/METHODS: We reviewed the medical records and imaging studies of all patients who underwent a computed tomographic (CT) angiogram and subsequently underwent CAS. The thoracic component of CT angiogram images were reviewed by an independent reviewer to determine presence of any calcification and severity of calcification was graded as follows: mild, single small calcifications; moderate, multiple small calcifications; severe, one or more large calcifications. The difference between small and large calcification was based on whether the calcification was larger than the radius of the aortic arch.
RESULTS: Aortic arch evaluation was performed in 77 patients (mean age卤SD of 70卤10 years). Aortic arch calcification severity was graded as none/mild (n=4), moderate (n=29), and severe (n=31). Compared with patients with none/mild calcification, the mean procedure time (minutes 卤 SD) was significantly higher among patients with severe aortic arch calcification (134卤44 versus 82卤19, p=0.1). The mean fluoroscopic time was also significantly higher among CAS patients with severe aortic calcification (45卤20 versus 23卤4, p=0.06). Patients with severe aortic arch calcification had lower rate of favorable outcome (mRs 0-2) compared to those with mild/none calcification (67% versus 100%, p= 0.05).
CONCLUSIONS: Assessment of severity of aortic calcification prior to CAS using CT angiogram may help in risk stratification for procedural complexity and post-procedural outcomes.
