Several studies have shown that low levels of cholesterol, triglyceride, and LDL are independently associated with an increased risk of mortality in patients with ICH. Studies have also shown that there is an inverse relationship with cholesterol, triglycerides, LDL, and the incidence of ICH.
A retrospective study was performed on consecutive patients with spontaneous ICH from July 2009 to June 2012 at a community teaching hospital. The ICH score at admission, initial and 24 hour GCS, mRS, and complete lipid panel were evaluated; those who did not have these values were excluded. All factors mentioned were then compared to lipid panel levels. Patients were grouped for high and low cholesterol (cutoff=200 mg/dL) and LDL (cutoff= 100mg/dL) and the mean ICH score, initial and 24 hour GCS score, and mRS were compared. Correlations and means were computed between groups using SPSS software version 20.
Of the 265 ICH patients, 261 (mean age = 68.91 +/-14.40, 54% male) met the inclusion criteria. Patients with high cholesterol had a significantly lower ICH score (p < .05), and higher initial GCS score (p < .05). Patients with high LDL levels had a significantly lower ICH score (p < .05) and mRS score (p < .05), and a higher initial GCS (p < .05) and 24 hour GCS (p < .05). Our data suggests that increased cholesterol and LDL levels have a protective effect in patients presenting with ICH. Further prospective studies are warranted to corroborate our findings.
