Title Standardized Assessment of Concussion (SAC) in Active Professional Boxers

Objective

Background The SAC represents a brief sideline examination to assess orientation, immediate memory, concentration and delayed recall in concussed athletes.

Design/Methods Eighty-eight consecutive professional boxers who were applying for license to box in New York State were screened with a modified version of the SAC. Boxing records were reviewed for total number of bouts, number of losses, duration of boxing career, average number of rounds sparring per week, number of losses by knockout (KO) or technical knockout (TKO), average estimated weight loss prior to a bout, age at starting boxing and duration of boxing career. Boxers with a SAC score of < 25 were compared to those with a SAC score of > 25.

Results The mean SAC score for 88 active boxers was 26.7 + 2.6. There was no correlation between SAC score and exposure (total number of fights, number of losses, losses by KO/TKO, years as professional boxer, sparring rounds per week, and number of losing rounds). There were no statistically significant differences between the groups (SAC < 25 and SAC > 25) except for the age at the start of boxing and estimated average weight loss prior to a bout. Boxers who scored < 25 on the SAC started boxing at an average age of 21.8 years compared to 17.5 years among those with a score of > 25 (p=0.041). Boxers who scored < 25 on the SAC estimated a mean weight loss of 5.5 pounds prior to a fight. This compared to 7.5 pounds in those who scored > 25 (p=0.046).

Conclusions The SAC can serve as screening tool for the pre-competition evaluation of cognitive function. However, there were limited correlations with boxing exposure.
