Brucellosis Presenting as a Longitudinally Extensive Transverse Myelitis

We report a case of a 44 year old previously healthy man who presented for left sided numbness. His history goes back to 2 weeks prior to presentation when he started to have a progressively increasing numbness over his left side which was associated with gait ataxia and urinary urgency without any history of transient neurological deficits. His exam was pertinent for weak left forearm extensors and hip flexors, hyperreflexia in both patellae, and sustained ankle clonus on the left. His magnetic resonance imaging (MRI) of the brain and cervical spine revealed a cervical non-enhancing intramedullary lesion extending from the level of C3 to C6, causing edema of the spinal cord, with associated meningeal enhancement extending from the brainstem to the lower cervical level.  An extensive blood and cerebrospinal fluid (CSF) battery of tests showed csf pleocytosis with a neutrophilic shift, hypoglycorrachia, an elevated csf protein level of 4.92 g/dl, and positive Brucella titers of more than 1:640 in blood and csf with negative anti-aquaporin 4 antibodies, oligoclonal bands, toxocara canis antibodies, viral and tuberculosis polymerase chain reactions, bacterial culture, vasculitic panel, and vitamin levels. Patient was started on ceftriaxone, rifampicin, and doxycycline for a 6 month course.
