Thalamic subregions and visual function changes in NMOSD and RRMS patients using 7T MRI

Investigate thalamic subregional volumes using 7T MRI in neuromyelitis optica spectrum disorders (NMOSD) and relapsing-remitting MS (RRMS) patients, and their relationships to visual function (VF) measures.


Atrophy in deep gray matter (DGM) structures in NMOSD patients, especially in the thalamus, is uncertain due to conflicting results, unlike in RRMS. Relationships of DGM volume and VF measures (visual acuity [VA]) are of particular interest in NMOSD given the predilection for optic nerve involvement.

 

 

 

7T 3D magnetization prepared rapid acquisition gradient echo MRIs were acquired in age- and sex-matched healthy controls ([HC] n=10), RRMS (n=10), and aquaporin-4 seropositive NMOSD (n=10) patients. Parcellation of DGM structures was conducted using an atlas-based approach to identify thalamic subregions and other DGM structures. VF was determined with high-contrast VA decimal score.

 

 

 

No difference was found when comparing whole thalamic volume between HC, RRMS and NMOSD patients (HC: 17582.3±1658.5mm3, RRMS: 16532.2±1873.4mm3, NMOSD: 17031.0±1316.4mm3, p=0.119 and p=0.534 for HC vs. RRMS and HC vs. NMOSD, respectively). Comparisons with HC thalamic subregions and other DGM structures showed that NMOSD patients had no significant changes. RRMS patients showed significant decreases in the lateral geniculate nucleus ([LGN] HC: 214.8±30.8mm3, RRMS: 185.1±35.5mm3, p=0.02) and subthalamic nucleus (HC: 437.4±34.8mm3, RRMS: 396.0±57.8mm3, p=0.03). Linear mixed-modelling with VA revealed correlations with LGN (β=0.559, p=0.02) and subthalamic nucleus volumes (β=0.569, p=0.03) in a subcohort of NMOSD (n=9) and RRMS (n=1).

 

 

 

No volume differences were found in this preliminary analysis of the NMOSD thalamus, while there were several decreased subregions in RRMS, compared to HC. Despite no significant decrease in the LGN or subthalamic nucleus in NMOSD vs. HC, their volumes associated with VA; suggesting that changes occur in the nuclei and affect VF in NMOSD. Larger sample sizes are required to elucidate true volume differences in thalamic subregions, and associations between MRI and VA measures.
