Impact of Race on Mechanical Thrombectomy Safety Outcomes

To describe any difference in mechanical thrombectomy (MT) safety outcome among different races.
The prevalence and popularity of MT in acute ischemic stroke continues to increase, however there continues to be a significant rate of complications in a certain percentage of patients. Despite known differences in ischemic stroke presentation and incidence among races it is unclear if there are also differences in MT safety outcome.
A retrospective analysis was performed in consecutive patients undergoing MT from 2/7/2015 to 6/29/2017 in a large tertiary care center. Data was collected by independent chart review of the electronic medical record. Multinomial logistical regression analysis was performed.
The cohort included 180 patients, median age 73y, 55% women. 21% were Caucasian, 45% Hispanic Caucasian, 27% African American, 7% other. Multiple metrics of safety outcomes were assessed, none of which met statistical significance. These included severe disability (Modified Rankin Score >=5 (p=0.66)), hemorrhagic conversion following procedure (p=0.39), subarachnoid hemorrhage following procedure (p=0.50), TICI score <= 2a (p=0.91), worsening NIHSS following procedure (p=0.41). Although not statistically significant, assessment of worsening NIHSS and disability showed increased OR in other races when compared to Caucasians (OR 1.35 to 2.21 in worsening NIHSS). Overall there was no difference in efficacy outcomes by race.
There is no statistical difference between non-Hispanic Caucasians, Hispanic Caucasians, and African American with respect to safety outcomes in the sample studied. A larger multicenter analysis is needed for further analysis.
