One Year Follow-Up after Elective Carotid Artery Stenting in 720 Patients: A Single Centre Experience

The study aimed to present the experience of performing elective carotid artery stenting(CAS) over 15 years by a single operator without using any protection devices and evaluating one year follow-up of patients.
Literature stating clinical efficacy of CAS with use of cerebral protection devices has been widely available. But data on CAS without protection devices are scarce. 
This was a single-centre, retrospective, observational study conducted at a Tertiary care hospital in India between March, 2002 and March, 2017. Total 720 patients who underwent CAS without using any protection devices were included in study. The interventional procedures were carried out as per standard protocol.

Mean age of patients was 59.27 ± 9.80 years. Male proportion was 71.5%. Diabetics and hypertensives were 43 and 82%, respectively. Coronary artery disease was co-existent in 58% patients. Previous transient ischaemic stroke(TIA) and previous stroke was observed in 62% and 18.4% patients, respectively. Mean diameter stenosis was 88% with a range of 75 to 99%. Right, left and bilateral internal carotid artery was stented in 60%, 36% and 4%, respectively. 750 stents were implanted in 720 patients.

Procedural success was 99.7%. Two cases could not be done due to severe tortuosity with marked calcification of the carotids. Upto one year follow-up, one patient had stroke, recovered partially and 5 cases had TIA, recovered completely with no sequelae. Ten patients had groin haematoma with 1 patient requiring blood transfusion. One patient needed femoral vascular repair, 1 patient died due to MI following coronary bypass surgery which was conducted 2 months after carotid stenting.

Carotid artery stenting without use of any protection device is safe and effective in patients with significant carotid artery stenosis, with low rate of complication at one-year follow-up.
