Neurophysiological Correlates of Excitability in ALS: Pretreatment Findings from the Retigabine and Mexiletine-2 Studies

To demonstrate whether pretreatment measures of transcranial magnetic stimulation (TMS) and threshold- tracking nerve conduction studies (TTNCS) in familial and sporadic ALS subjects enrolled in the Retigabine (RET) and Mexiletine-2 (MEX2) trials differ from those of healthy volunteers in the RET study.

 

TMS and TTNCS have previously suggested hyperexcitability of cortical motor neurons and peripheral motor nerve axons in ALS subjects, potentially contributing to pathogenesis.

 

ALS subjects, age-matched healthy controls (MHC), and unmatched healthy controls (UHC) were evaluated at screening or baseline pretreatment (first good-quality assessment). TMS measures included short-interval intracortical inhibition (SICI), resting motor threshold (RMT), slope of input/output curves (I/O), and cortical silent period (CSP). TTNCS assessed strength-duration time constant (SDTC), depolarizing threshold electrotonus, 90-100 ms (Ted), and recovery cycle superexcitability (SupEx) and late subexcitability (SubEx2).

 

138 ALS subjects, 13 MHC, and 57 UHC were evaluated. The average SICI, RMT, and I/O curve pretreatment were significantly higher for ALS subjects using oneway ANOVA (SICI: 0.95±0.72 vs 0.69±0.65 vs 0.38±0.29, p<0.001; RMT: 65.6%±21.5 vs 54.0%±11.6 vs 50.2%±9.0, p<0.001; I/O: 12.8±5.2 vs 10.2±4.0 vs 8.9±4.1, p<0.001) and lower for CSP (84.8ms±37.5 vs 87.8ms±46.7 vs 107.0ms±23.9, p =0.002). The average SDTC, Ted, and SupEx were significantly higher for ALS subjects (SDTC: 0.497ms±0.099 vs 0.450ms±0.070 vs 0.451ms±0.090, p=0.010; Ted: 47.0%±9.0 vs 47.5%±4.5 vs 43.3±5.5, p=0.016; SupEx: -28%±7.7 vs -26%±3.9 vs -24%±5.7, p=0.002) and lower for SubEx2 (26.7%±10.6 vs 30.0%±3.7 vs 31.8%±10.2, p=0.016).

 

The TMS findings in this cohort, especially the higher average SICI in ALS subjects, largely support increased excitability of cortical motor neurons in ALS. The unexpectedly high RMT in the ALS cohort may reflect the variable nature of this parameter as shown previously. The TTNCS findings, especially the higher average SDTC and Ted in ALS subjects, substantiate greater excitability of peripheral motor nerve axons in ALS.
