Angio-OCT micro vascular markers and central auditory disorder as indipendent predictors of cognitive impairment in the Great Age Study population

To assess if the clinical use of eye and ear biomarkers could be modifiable predictors of the  cognitive impairment
Increasing evidences supported the clinical use of the sense organs assessment in the prediction of cognitive impairment.Optical Coherence Tomography with angiography (OCTa),could represent an in vivo model of the of peripheral neurodegenerative processes,that could be generalized to the cortical systems. In a similar way, the study of the hearing functions,especially central auditory disorders (CAD), is considered a useful way to assess the risk of cognitive decline
We studied 65+ participants (N=582),from the community-based Great Age Study. We performed OCTa scan of both eye (OptoVue,AngioVue™) to measure deep retinal plexus vessel density (%). Because of the numerosity of  output variables we used principal components (PC) analysis  to describe at least 65% of the whole information using three eigenvectors (PC1,2,3). The CAD assessment is defined with the Synthetic Sentences Identification score with Ipsilateral Competitive message (SSI-ICM). Global cognitive functions were assessed with the Mini-Mental State Examination (MMSE) and the global frontal lobe functions with the Frontal Assessment Battery (FAB). We performed linear regression models on cross-sectional data to study the association between the PCs of Vessel Density measures, SSI-ICM and the cognitive measures

The PC1, that summarizes global vascularization of deep retinal layer,is associated with the MMSE (p=0.015) and FAB (p=0.002). The SSI-ICM score is strongly associated with MMSE and FAB (p<0.001).Worse sense organs status is predictor of poor cognitive functions.The coefficient of determination is really high both in the FAB  (R2= 0.41) and in the MMSE (R2=0.31) regression models

 

 

The retinal vascularization and central auditory biomarkers proposed are independent diagnostic predictors of the cognitive impairment both in the global and the frontal functions.
The assessment of deep retinal vascularization and central auditory functions are important markers of cognitive ability among elderly subjects.
