Long-term Follow up of Epilepsy Patients After a Single Neurologist Visit

To determine long-term outcomes of an epilepsy outreach clinic that provides a one time neurologist consultation for people with epilepsy (PWE).
Shortage of doctors and poor access to care are experienced by PWE in LMICs. Alternate models of care including outreach clinics should be explored.
A cross-sectional observational study was conducted from March 2016 to August 2018. Outcomes of consecutive PWE who had consulted in fifteen outreach clinics conducted on the Lifeline Express (LLE) between 2009 to 2014 were compared with outcomes of PWE registered in the same duration at a tertiary care clinic of All India Institute of Medical Sciences, New Delhi (AIIMS). The primary outcome was to determine if patients were still continuing AEDs.  Epilepsy resolution, premature stopping of AEDs and death were also noted. Patients from the LLE clinic were contacted telephonically to determine their current status. For the AIIMS clinic, patient records were reviewed and phone calls made to PWEs who had not followed-up for >1 year.
A total of 1923 and 1257 patients were enrolled from the LLE and AIIMS clinics respectively. Long-term outcomes were available for 688 AIIMS patients and 531 LLE clinic patients. Of the AIIMS clinic patients, 581(87%) were continuing AEDs, 49(7%) had discontinued AEDs as advised, 39(6%) had discontinued AEDs without advice and 19(2.8%) were dead. Outcomes in 531 LLE patients revealed that 351(72%) continued to be on AEDs, 34(7%) had discontinued AEDs on advice, 106(22%) had discontinued AEDs against advice and 40(7.5%) were dead. Inability to contact a significant number of patients from both clinics: 544 (44%) from AIIMS and 1314 (71%) from the LLE clinic; was a major limitation of the study.
In LMICs, epilepsy outreach clinics may be improve access to care. Patients are likely to continue and benefit from treatment even after one consultation with a neurologist.
