Pilocytic Astrocytoma in an Elderly Patient with Eleven Years of Follow-up: A Case Report and Commentary on Management of PA in An Elderly Patient

Pilocytic astrocytoma (PA) is a low-grade glioma observed principally in children and young adults. Given the benign nature of most PAs, surgical resection without adjuvant therapy is the standard initial course of treatment. Little data are available to evaluate the applicability of this conservative approach to rarer manifestations of PA in older individuals.

We present the case of a 58-year-old woman with a five-year history of progressive, bilateral lower extremity numbness extending from the lower ankle to the anterior thigh. MRI of the cervical spine revealed a spinal cord tumor around the C2-C6 level. During a C6-T12 laminectomy, the neoplasm was partially resected and biopsied. Histopathological evaluation was consistent with pilocytic astrocytoma. The procedure was not accompanied by adjuvant therapy, and the patient remains stable with minimal progression of symptoms or tumor size through more than eleven years of follow-up.
This is a rare case of a late quinquagenarian treated for PA with over a decade of follow-up. The long-term success of her treatment course suggests that PA in the older population may be of a similarly-benign character as that in the younger demographic, and therefore may warrant a similarly-conservative approach to management.
