Using Smartphone Mindfulness Apps to Increase Trainee Resilience and Reduce Burnout

Evaluate feasibility and benefit of mobile delivery of mindfulness training on trainee burnout and resilience.

Programs which prevent burnout and integrate wellness enhance trainees' ability to provide quality patient care and fulfill the six core ACGME competencies.Mindfulness interventions improve attention and compassion and reduce rumination.However, busy trainee schedules limit accessibility.Mobile technology is a convenient and self-regulated delivery platform.
47 new residents and fellows from a single center were randomized to receive a one-time lecture on mindfulness and 6-month smartphone app subscription delivering on-demand 10-minute mindfulness meditations, or lecture only.Burnout and resiliency were measured at baseline and 3- and 6-months post-intervention, using an online abbreviated Maslach Burnout Inventory, resiliency survey, and Kessler Psychological Distress scale.Statistical analysis was performed with 2-tailed paired t-tests, ANOVA and Tukey HSD.
26 participants(55%) responded to the 3-month survey and 23 participants(49%) completed the study at 6 months.Resilience significantly increased by mean 6 points only in the intervention group at 6 months(p=0.0330, 95%CI(0.6, 11.4)),with no significant change in burnout or psychological distress.Participants used 20-300 minutes of mindfulness meditation over 6 months.Compliance among responders was 50% at 3 months and 92% at 6 months. In the intervention group at 3 months, those who used the app had higher baseline personal achievement (15vs13, p=0.0244) and resilience (75vs63, p=0.0032) compared to those who did not.Factors limiting compliance were identified by trainees as “no time”(75%), “too tired”(50%), “already engage in other stress-reducing activities”(31%) and “no motivation”(13%).80% felt that protected time and program-led mindfulness sessions would increase compliance.
Mobile mindfulness interventions can increase trainee resilience.Smartphone apps have the potential to be effective delivery mediums.Compliance is still limited by multiple factors.Ironically, trainees who are less resilient and more burnt-out are less motivated to use the app.Consider program-led initiatives and protected time to target mobile mindfulness interventions at less resilient and more burnt-out trainees.
