The Impact of a Dedicated Emergency Department Neurology Resident on the Inpatient Neurology Service

To evaluate the impact of the ED rotation on the inpatient neurology services.
University Hospital is a tertiary care 500-bed hospital with a Comprehensive Stroke Center and a Level 1 Trauma Center. Our inpatient neurology service consists of 2 teams: a stroke and a general neurology service. We obtained funding for a neurology resident to staff the Emergency Department (ED) for 50-60 hours per week.
Anonymous surveys were completed by faculty and senior residents on the stroke, general and ED services from July 1-October 8, 2018 for a total of seven 2-week rotations. The ED rotation was evaluated and ranked on a five-point scale from strongly disagree (1) to strongly agree (5). A response was considered positive (agree or strongly agree) when the response was > 4. 

6/8 (75%) of faculty and 19/19 (100%) of residents completed the survey. Faculty responded most favorable and uniformly agreed that the ED rotation is helpful (5.0) and response time to evaluate an ED patient has decreased (4.8). They agreed that rounds are uninterrupted (4.5) and noted a smooth transition of patients from ED to floors (4.5). The general and stroke residents responded positively to the ED rotation (4.8) and felt less burdened (4.6). The stroke residents were most dissatisfied on having enough exposure to new patients (4.0). The ED residents reported a good work-life balance (4.7), a manageable overall workload (4.6) and were most dissatisfied and most divided about gaining enough clinical experience by seeing a sufficient number of patients (3.9). The ED residents strongly agree on improved ED patient disposition time (4.6).

There was a decline in the burden of the residents on the floor services, improvement in ED patient transit time, and rounds were uninterrupted. Residents on the stroke service noted an impact on the exposure to new patients
