To determine if timing of National Board of Medical Examiners (NMBE) Shelf exam and Objective Structured Clinical Examination (OSCE) within an academic year makes any difference in medical student grades.

At the University of Massachusetts, a 3-week Neurology clerkship is integrated with Internal Medicine in a 16-weeks care of adults thematic rotation.  A composite score of shelf exam, OSCE and preceptor evaluation is used to calculate a final grade. OSCE and shelf exam are administered toward the end of the 16-week rotation. However medical students rotate through the clerkship at different times within the 16-week rotation and within the academic year. An important QI question is whether students shelf exam and OSCE grades are affected by the time of their rotation.

245 medical student neurology clerkship evaluation scores from academic years 2016 to 2018 were compared longitudinally in an academic year - block 1 (May to August), block 2 (September to November), block 3 (January to April) – and within the 16-weeks clinical rotation – block A (weeks 2-4), block B (weeks 7-9), block C (weeks 12-14) to determine if there was a difference in scores between these groups.
There was no difference in OSCE scores between block 1-3 (78.9±0.2, 80.1±0.3, 80.7±0.3, p < 0.07) and blocks A-C (79.6±0.3, 79.4±0.2, 80.6±0.2, p < 0.24), and shelf scores between block A-C (79.4±58.0, 78.6±62.3, 78.8±61.1, p < 0.77, ANOVA). There was only a difference in shelf score between block 1-2 (80.2±51.0, 77.0±70.8, p<0.01), and block 2-3. (77.0±70.8, 79.7±54.8, p<0.03, student’s t test). Further analysis is currently being conducted to determine if correction for gender and step 1 score has any effect on the significance of these results.
The timing of objective clinical testing within an academic year has minimal difference in student performance score.
