Trends in the Incidence of Migraine in Patients Hospitalized for Acute Ischemic Stroke in the United States Using the Nationwide Inpatient Sample Database

Several studies have reported a strong association between incidence of ischemic stroke in the migraine patient population, but data estimating the in-patient hospitalization with migraine in this population remains sparse.
The Nationwide Inpatient Sample (NIS) database was analyzed from years 2006-2014. The International Classification of Disease-Ninth Revision (ICD-9) was used to identify ischemic stroke related admissions and migraine (ICD CM code 346.xx). P-values for trends were generated by the Cochrane-Armitage test for categorical variables and simple linear regression for continuous variables.
We identified 3,922,444 hospitalizations with a primary diagnosis of ischemic stroke of which 60,159 (1.53%) patients had a history of migraine (Mean age 56.3 ± 15.4 years, 73 % Male, 66.7 % White). The highest incidence of migraine in this population was observed in the southern states (40.3%) of the USA, which was associated with lower in-hospital complications and lower comorbidity index (34% vs 49%). The incidence of migraines in this population showed a statistically significant rising trend (relative increase of 114 %) with an increasing trend in overall cost of hospitalization (relative increase of 129%). We observed a declining trend in length of stay associated with migraine hospitalization (relative decline of 11.4%).
Our study shows an increasing trend of ischemic stroke associated migraine hospitalization with an upward trend in overall cost of hospitalization. Southern states showed highest incidence of migraine for unclear reasons but was associated with a lower incidence of in-hospital complications and comorbidities.
