LIFEware Life Satisfaction and Pessimism are Correlated with the Beck Depression Inventory-Fast Screen and with Disability Progression in Individuals with Multiple Sclerosis

To investigate the correlations between single-item psychosocial LIFEware™ metrics and the Beck Depression Inventory-fast screen (BDI-fs) in people with MS (pwMS).
Depression is an extremely common comorbidity in multiple sclerosis (MS). The LIFEware™ system is a short, patient-reported outcomes (PRO) questionnaire assessing physical and psychosocial limitations that includes several single-item measurements associated with mood.
MS patients were extracted from the prospective Cardiovascular, Environmental and Genetic (CEG) study. The LIFEware™ psychosocial measures were completed by 134 pwMS who simultaneously completed the validated BDI-fs. Pearson correlations were calculated between the BDI-fs and the following LIFEware™ measures: life satisfaction, loneliness, pessimism, tension, panic, annoyance, morbid mood and guilt. Correlations between the LIFEware™ psychosocial measures, BDI-fs and the Expanded Disability Status Scale (EDSS) were also calculated.
LIFEware™ pessimism, loneliness and life satisfaction were most strongly correlated with BDI-fs (r2=0.56, 0.45 and -0.50, respectively; all p-values<0.001). When stratified by sex, life satisfaction in males was most strongly correlated with BDI-fs (r2=-0.66, p-value<0.001), while pessimism in females was most strongly correlated with BDI-fs (r2=0.55, p-value<0.001). Similar results were identified in aging pwMS (≥55 years of age) where both life satisfaction and pessimism were most strongly correlated with BDI-fs (r2=-0.63 and 0.69, respectively, both p-values<0.001). When examining correlations with disability progression, the BDI-fs, LIFEware™ life satisfaction, and pessimism were correlated with EDSS (r2=0.18 for the BDI-fs, r2=-0.27 for life satisfaction, and r2=0.17 for pessimism, all p-values<0.05).
LIFEware™ psychosocial measures, particularly life satisfaction and pessimism, and the BDI-fs are highly correlated in pwMS. These measures were also statistically significantly, though modestly, correlated with disability progression.
