Disability or Disease: What Stands at the Basis of Connectivity Patterns within the Visual Network following Damage to the Optic Nerve?

To study connectivity patterns within the visual cortex following optic neuritis (ON) and to test whether visual network behavior following damage to the optic nerve is driven by disease type or by form of disability.
We have previously shown that functional modification in the visual cortex of clinically isolated ON (CIS-ON) patients may occur to compensate for the damage sustained by the affected optic nerve. Now, utilizing graph theory analysis methods we delve into the individual regions of the visual system and expand our study to include neuromyelitis optica spectrum disorder (NMOSD) patients, comparing both disease and disability. 
Eighty-eight subjects, including 18 CIS-ON, 21 CIS without ON (CIS-nON), and 23 NMOSD, as well as 26 healthy controls (HC) underwent resting state functional MRI (RS-fMRI). Visual networks were constructed through inter-regional temporal correlations of RS-fMRI time-courses, comprising 50 visual regions (25 per hemisphere).
No significant differences were found between hemispheres in any of the groups. Both hemispheres show that the HC visual networks are significantly denser than the CIS-nON group and that the CIS-ON and NMOSD groups are similar in overall network structure. Measures of network segregation show similarities between CIS-ON and CIS-nON and between HC and NMOSD, particularly in higher regions, as do efficiency parameters.
The internal structure and efficiency appear to be more affected by disease type, with similarities between both CIS types and between NMOSD and HC, particularly in higher regions.  The overall structure of the visual network, however, appears to change according to disability, and so when damage occurs in the visual system (NMOSD, CIS-ON), attempted compensation takes place, which is not seen when other systems are damaged (CIS-nON). Therefore, we suggest, that compensatory mechanisms are dependent on disability rather than disease.
