Ocrelizumab Safety and Effectiveness in the One Year Treatment of Multiple Sclerosis Compared to Other Disease Modifying Therapies

To describe the patient one year experience of Multiple Sclerosis (MS) patients treated with ocrelizumab (OCR).
OCR, used in the treatment of MS, is a monoclonal antibody targeting CD20, resulting in B-cell depletion.

100 consecutive MS patients prescribed OCR at the University of Colorado Hospital were retrospectively followed for up to one year from OCR start date. Lab data, relapse history, adverse events, MRI outcomes, disease history and patient characteristics were collected. Descriptive statistics were used to describe the sample group. No progression-related outcomes were assessed.

Patients had a mean age of 44.7 years at date of first infusion; were predominantly female (73.0%); and had a mean MS disease duration of 10.2 years. Of the sample group, 80 (80.0%), 16 (16.0%), and 4 (4.0%) were relapsing-remitting, secondary progressive, and primary progressive MS, respectively. Ten (10.0%) patients experienced lymphopenia ≤500/mm3, and one (1.0%) experienced neutropenia ≤1000/mm3 with a level of 800/mm3. Two (4.6%) experienced IgG levels ≤500, 6 (14.0%) experienced IgM levels ≤40. Infections resulting in an emergency department visit occurred in 8 (8.0%), none of whom were hospitalized. During the first and second infusion, 9 (9.2%) and 7 (8.6%) experienced an infusion reaction that interrupted the OCR infusion, respectively, and none experienced a life-threatening reaction. Two (2.0%), zero (0.0%), and five (8.6%), experienced a clinical relapse, enhancing lesion and new T2 lesion, respectively. Nine (9.0%) patients discontinued OCR at >12 months, six (6%) due to insurance problems, two (2%) due to family planning and one (1%) due to patient’s concern of developing cancer. 
Our data suggests OCR is safe and effective in the treatment of MS. Further analysis of additional patients and CD20/CD19 absolute values to examine reconstitution will be conducted. One year effectiveness will be compared to similar data with rituximab, natalizumab, fingolimod and dimethyl fumarate.
