Vascular dementia in elderly .Population-based study in Matanzas.

PURPOSE: To evaluate vascular dementia (VD) according to clinical and epidemiological variables and the association with risk factors in older adults 

INTRODUCTION: Dementia is a major public health problem. There is an exponential increase in the number of demented people each year. Someone develops a dementia every three seconds. The long duration of the disease, high costs and mortality rates have made this a public health priority to the World Health Organization (WHO).

 METHOD: A cross-sectional, population-based "door-to-door" study was conducted in 901 people over 65 years old living in a chosen area of Matanzas city, from 2003-2007 as a part of Cuban Health and Aging Study (CHAS). The prevalence and standardized morbidity rates (crude and adjusted) were calculated for the studied chronic diseases having 95% of confidence intervals, using a Poisson regression model and indirect standardization. A multivariate analysis and Pearson correlation were performed to dichotomous variables to calculate risk association.
RESULTS: Dementia was presented in 101 individuals, with a prevalence rate of 11.2% for total dementia (n = 901),5.21% for Alzheimer disease (AD) and 2.33% for vascular dementia (VD). Women and people aged 74 and over showed higher prevalence. Hypertension (OR 5.17 CI 95% 1.49-17.97) and physical inactivity (OR 6.38 / 95% CI 1.62-25.05) were a risk factor to  VD. Abdominal obesity and alcoholism were not frequently associated. Prevalence of VD adjusted by age, gender and educational level was higher in individuals aged 80 and over (47.61%, 95%CI 24.32-70.91) and female (57.14, 95% CI 34.06-80.22). Less than 5% of demented people were illiterate (4.76 95% CI 5.17-14.69).

 CONCLUSIONS: The prevalence of VD in the studied elderly people is high and exceeds other reports.
