Prevalence of Atherosclerotic Vertebrobasilar Disease in patients with Ischemic Stroke in an Argentinian cohort of patients

To describe the prevalence and clinical/demographic characteristics of VBA in an Argentinian population.
Posterior circulation stroke (PCS) represents about 20% of all strokes.  Vertebrobasilar atherosclerosis (VBA) is the mainly cause of artery stenosis. Nevertheless the prevalence and clinical characteristics of VBA (asymptomatic and symptomatic disease) in the general population with stroke is not well known. VBA have received less attention than internal carotid atherosclerosis (ICA).
All consecutive patients admitted with stroke during a period of 18 months were evaluated. We analyzed the clinical and demographic characteristics. We compared symptomatic VBA (PCS and >50% of stenosis) vs asymptomatic VBA (different vascular territory or <50% of stenosis) and symptomatic VBA vs symptomatic ICA. Statistical analysis: Fischer test (qualitative variables) and Mann-Whitney test (quantitative variables); p<0.05 was considered significant.
We included 125 patients: 69.6% anterior circulation stroke (ACS), 27.2% PCS and 3.2% both (ACS+PCS). VBA: 25 (20%) patients; VBA associated with ICA: 84% of cases. Five were symptomatic: 3 patients had intracranial and 2 had extracranial stenosis. Sixteen men (64%); mean age: 72,56 years. Clinical characteristics: 22 (88%) hypertension, 13 (52%) smoking, 13 (52%) hyperlipidemia, 8 (32%) diabetes mellitus, 3 (12%) ischemic cardiopathy, 4 (16%) atrial fibrillation and 3 (12%) previous TIA/Stroke. Admission NIHSS: 6.32; in-hospital length: 8.58 days. All the clinical characteristics were similar to the general population. The comparison between: symptomatic vs asymptomatic VBA and symptomatic ICA vs symptomatic VBA did not show any statistical differences.
One every 5 patients in our population with stroke had VBA and most of them had concomitant ICA too. No differences were observed in clinical characteristics  of VBA group regarding the presence or not of symptoms. Moreover, their clinical profile was similar to  symptomatic ICA. To our knowledge no VBA larger studies in Hispanic populations had been previously performed.
