Neurofilament Light Chain Protein Levels in HIV Positive Subjects with Neuropathy

To determine if neurofilament light chain (NFL) is elevated in patients with HIV associated neuropathy.
NFL protein is a structural component of myelinated axons, and has been proposed as an indicator of axonal injury in several neurodegenerative conditions. It has also been shown to be elevated in the cerebrospinal fluid (CSF) of subjects with HIV associated neurocognitive disorder. NFL has not been previously evaluated in HIV associated neuropathy.

54 HIV positive subjects in the Johns Hopkins HIV Neurology cohort were evaluated with a medical history, neurological exam including assessment of neuropathy, neuropsychological test battery, serum analysis and lumbar punctures. CSF and plasma NFL concentrations were analyzed using in house ELISA and Single molecule array (Simoa) methods, respectively. Median NFL comparisons by neuropathy presence and severity were made using Kruskal-Wallis test, with significance set at p<0.05.

26/54 patients had neuropathy, (18-mild, 6-moderate, 3-severe). There was a trend towards increasing concentrations of CSF and plasma NFL with increasing neuropathy severity. (CSF NFL: no neuropathy = 578.3, mild neuropathy= 588.0, moderate neuropathy= 631.0, severe neuropathy= 929.5; plasma NFL: no neuropathy= 6.3, mild neuropathy= 7.3, moderate neuropathy= 7.7, severe neuropathy= 8.4). Median CSF NFL was significantly elevated in HIV dementia patients compared to those without dementia (803 vs. 576, p=.032). Adjusting for presence of HIV dementia, there was no difference in CSF or plasma NFL with increasing neuropathy severity.

An interesting positive trend between neuropathy severity and both plasma and CSF NFL concentrations was found in our cohort of HIV patients, but results did not reach significance partly due to our small sample of patients with moderate and severe neuropathy. These data suggest that NFL concentration may be useful to evaluate active injury due to HIV not only in the central nervous system, but also in the peripheral nervous system.
