To employ various interventions with neurology residents in order to improve their documentation practices and in turn increase neuro-hospitalist division revenue.

Neurohospitalist divisions are shown to improve outcomes for inpatients with neurological disease. These services can be financially challenging given heavy reliance on accurate documentation. An incomplete comprehensive neurological exam is the most common deficiency. At teaching hospitals documentation is heavy reliant on residents who do not receive adequate training on documentation and billing. This leads to incomplete documentation and lost income.

Intervention 1 included adding small reminders to the EMR’s built-in templates in key areas prone to deficiency. Intervention 2 included formal education with residents regarding documentation, billing and coding. Intervention 3 included a more comprehensive note template rework. Data from initial inpatient and consult E/M codes were captured and pooled for one quarter before and after each intervention. Pre and post-intervention revenue was compared for statistical significance.
Major and minor template changes showed no significant impact on total billing. Resident education was associated with a significant increase in the proportion of Level 3 initial inpatient visits the following quarter with an increase in quarterly billing of 12% as compared to only a 6% increase in volume (p: 0.01).
Billing as a neuro-hospitalist division remains a unique challenge. At teaching facilities, there is a heavy burden on residents to complete accurate documentation. Therefore, targeted interventions can yield measurable improvement to the bottom line.
