Changes in Brain Volume in Specific Regions could be related to Disability Progression in Primary Progressive Multiple Sclerosis Patients

To assess the relationship between brain volume changes in specific brain regions and disability progression in primary progressive multiple sclerosis (PPMS) patients.

Although several studies have been performed about brain volume changes in patients with relapsing multiple sclerosis (RMS), there is scarce information of the effect of the neurodegenerative process in regional or global atrophy in disability and cognition in patients with PPMS.

A prospective, observational study was conducted in adult patients with PPMS (McDonald 2010) from 11 MS Units in Spain. MR Imaging scans were processed using the tissue segmentation (white and grey matter) and the identification of 252 anatomic brain areas. Outlier samples were removed from statistical analysis. Regression and correlation analyses were performed to determine the relationship of anatomic areas and evidence of progression (EP) according to expanded disability status scale (EDSS) score or/and 9 hole-peg test (9HPT) or/and

Forty-four out of 55 patients were studied (mean age 55.8 (SD 9.5) years, 56.4% male). MS evolution mean time was 4.1 (SD 4.5) years and median EDSS score 5.5. Twenty (45%) patients had EP after 1-year of follow-up (1YFU). Baseline white matter (WM) adjacent to left amygdala (WMLA) and WM adjacent to right cerebellar tonsil (WMRCT) volumes showed differences between patients with EP and those with no EP (NEP) after 1YFU [(WMLA EP/NEP 0.56cm3/0.91cm3; P=0.01); (WMRCT EP/NEP 2.6cm3/3.1cm3, P=0.003)], as well as other 7 WM regions (P<0.05). Patients with a baseline cutoff point below 0.62cm3 (WMLA) and 2.9cm3 (WMRCT) were associated to a higher risk of EP after 1 year.

WMLA and WMRCT baseline volumes could be predictive of disease progression in PPMS patients after 1 year, as well as other WM specific regions.
